Dental Report
Dental reports for insurance claims and other purposes may be obtained from the National Dental Centre Singapore (NDCS)'s Dental Records Office. The release of dental information is subject to official approval by NDCS.
Who can request for a dental report?
- Patients aged 21 years and above.
- Patients aged below 21 years may apply for a dental report with the consent of their parent or legal guardian.
- Interested third parties, with the consent of the patient.
- If the patient is mentally incompetent or deceased, the written consent of their authorised representative or all their immediate next-of-kin must be obtained for the release of the dental report.

Ordinary Medical / Dental Report (Request via HealthHub)
- This is a written report that provides relevant facts about the medical condition, treatment, and medications the patient is receiving/has received.
- It is a factual report put up by the doctor based on patient’s existing medical records.

Specialist Medical / Dental Report
- This is a written report that requires the doctor's specific opinion and prognosis of the patient's medical condition.
- A new assessment and review of the patient may be required at the Specialist Outpatient Clinics. Consultation fees may be charged separately by the doctor on the day of the assessment.
- Additional charges will be borne by the patient if further tests such as radiological and laboratory investigation procedures are required.

Completion of Insurance Claim Form (Request via HealthHub)
- A medical insurance form questionnaire that requests for detailed information related to the treatment/surgery done for the patient.
- Requestor is required to furnish the insurance claim form which is provided by his/her insurance company.

Work Injury Compensation Form
- This is for the purpose of determining work-related injuries and the degree and period of disability.
- Scope of the report is as per "Medical Report on Traumatic Injuries for Workmen's Compensation" form prescribed by the Ministry of Manpower (MOM).
- A new assessment and review of the patient may be required at the Specialist Outpatient Clinics. Consultation fees may be charged separately by the doctor on the day of the assessment.
- Additional charges will be borne by the patient/employer if further tests such as radiological and laboratory investigation procedures are required.

Simple Insurance Form (With No Charting)
- A medical insurance form questionnaire that only requests for basic information such as final diagnosis, name of procedure and date of visits.

- Requires doctor to complete a tooth reference chart that is included in the form.

- Provides brief information (e.g. date of consultation with diagnosis).

Timeline for Request Completion
Requests will take four to six weeks to process.
We can only process your application upon fulfilling the verification and receipt of all necessary forms (with all fields completed), supporting documents and payment.
