Dental Report Request
How to apply for a Dental Report?
If you are making claims from your insurance company or if you require a dental report, you may either
Mail a completed copy of the Clinical Abstract Application Form, together with your cheque payment, to the address listed below
Visit us at the Central Records Office, Level 1, National Dental Centre Singapore.
Opening Hours: Mondays to Fridays, 8.00am to 5.15pm.
Who can request for a Dental Report?
Patients aged 21 years of and above
A patient who is a minor may apply for a dental report with the consent of his/her parent or legal guardian
In the case of a deceased or mentally incompetent patient, the written consent of his authorized representative or all his immediate next-of-kin must be obtained for the release of the dental report.
What are the charges?

Completion of Workmen Compensation Form
Simple Insurance Form (With No Charting)
Simple Insurance Form (With Charting)

Certified True Copy of Report
How do I make the payment?
Payment can be made using cash, credit card, cheque or NETS at the Central Records Office.
Cheques should be crossed, made payable to the 'NATIONAL DENTAL CENTRE SINGAPORE PTE LTD' and mailed to:

Please quote the name and NRIC of the patient on the back of the cheque. Please do not send in any cash by post.
Timeline for Request Completion
Please allow 4-6 weeks for processing of requests.
Delays in prompt processing may be result of:
Patient's upcoming clinic appointments
The doctor is away on leave
Collection of Report
Please bring along your NRIC when collecting the report
An authorization letter stating the collector's name and NRIC is required if he/she is collecting the report for you
If the patient is deceased, please bring along a copy of the death certificate
