Medication Safety Townhall - 15 November 2019

Featuring guest speaker Dr Shim Yuan Tying, Principal Clinical Pharmacist at the Singapore General Hospital (SGH), the town hall session for clinicians and nurses centered on novel oral anti-coagulants (NOACs) and their risk of bleeding.

Dr Shim shared useful and interesting information on the pharmacological actions (i.e. the effect of these medications in patients) of Dabigatran, Rivaroxaban and Apixaban, the peri-operative and post-operative management of patients who are prescribed the medications and herbs that increase their bleeding risk.

Dr Nattharee Chanchareonsook, Chairperson for the Centre's medication safety committee also provided staff with a report on the number of Category B medication errors for Q1 and Q2 FY19 – an error that occurred but did not reach the patient. She also announced the launch of NDCS's Monthly Index of Medical Specialities (MIMs).

Dr Shim Yuan Tying is part of the Inpatient Anti-Coagulation Service (IPACs) team, who plays an integral role in the review and monitoring of anti-coagulation drugs usage in wards. The team works with doctors daily to provide patients with an optimal, safe and recommended medication dosage.
