Care Partnership Programme (CaPP)

  NDCS Care Partnership Programme (CaPP)

Enhancing Oral Health and Holistic Care

Established in April 2017, the NDCS Care Partnership Programme (CaPP) aims to strengthen community partnerships and develop a robust primary care network. Through CaPP, patients with stable dental conditions are co-managed and discharged to community care partners, while those with episodic conditions receive timely access to dental care from primary care providers. This initiative aligns with the Ministry of Health's vision of moving "Beyond Hospital to Community" and underscores the importance of collaboration between tertiary institutions and community partners.

A Shift Towards Primary and Community Care

As Singapore shifts its healthcare focus towards Primary and Community Care for Healthier SG, strengthening partnerships with primary care becomes imperative. By enhancing collaboration with primary care providers, we can improve oral health outcomes and deliver holistic, seamless care to our ageing population.

Benefits of Collaboration:

1. Updates on Latest Treatment Guidelines: Collaboration with primary care facilitates the exchange of information on the latest treatment guidelines and protocols. This ensures that clinicians across different settings are informed about the most current evidence-based practices, leading to standardised and optimal care delivery.

2. Exchange and Alignment of Clinical Knowledge and Best Practices: Collaborative efforts allow for the exchange and alignment of clinical knowledge and best practices between NDCS and primary care providers. This exchange fosters a culture of continuous learning and improvement, ultimately enhancing the quality of care provided to patients.

3. Sharing of Treatment Philosophy for Clinicians: Through collaboration, clinicians have the opportunity to share their treatment philosophy and approaches to patient care. This sharing of perspectives promotes mutual understanding and alignment of goals, resulting in more cohesive and patient-centered care delivery.

Benefits for Patients:
