Dental Report

Dental reports for insurance claims and other purposes may be obtained from the National Dental Centre Singapore (NDCS)'s Dental Records Office. The release of dental information is subject to official approval by NDCS.

Required documents

  • Completed application form with patient's written consent.
  • Scan or photocopy of patient's NRIC (front and back view), passport or birth certificate if submitting request via post or email.
  • Scanned copies or photocopies of other supporting documents, e.g. marriage certificate, birth certificate and letters of administration, as proof of the representative's relationship to the patient.
  • Dental report fees (see table below).

Applications will only be processed upon receipt and verification of all necessary documents and appropriate payment.

You may apply for a dental report in person at:

Closed on Saturday, Sunday and public holidays

 Additional charges will be borne by the patient if further tests such as radiological and laboratory investigation procedures are required for the completion of the dental reports.

Payment modes

  • Cash, NETS, credit card
    • For in-person applications made at NDCS Dental Records Office

  • Cheques
    • Cheque should be crossed and made payable to "NATIONAL DENTAL CENTRE SINGAPORE PTE LTD".
    • Please write the full name and NRIC of the patient on the back of the cheque.


Who can request a dental report?

  • Patients aged 21 years and above.
  • Patients aged below 21 years may apply for a dental report with the consent of their parent or legal guardian.
  • Interested third parties, with the consent of the patient.
  • If the patient is mentally incompetent or deceased, the written consent of their authorised representative or all their immediate next-of-kin must be obtained for the release of the dental report.


Processing time

Requests will take up to 6 weeks to process.

Delivery modes
You may choose to receive your dental report by:

  • Post: additional mailing charges may apply for registered mail (local / overseas).
  • Self-collection: NRIC to be presented during collection.
  • Collection by representative: An authorisation letter and NRIC of the representative must be presented during collection.
