Dental Report

Dental reports for insurance claims and other purposes may be obtained from the National Dental Centre Singapore (NDCS)'s Dental Records Office. The release of dental information is subject to official approval by NDCS.

Who can request for a dental report?

Patients aged 21 years and above.
Patients aged below 21 years may apply for a dental report with the consent of their parent or legal guardian.
Interested third parties, with the consent of the patient.
If the patient is mentally incompetent or deceased, the written consent of their authorised representative or all their immediate next-of-kin must be obtained for the release of the dental report.

How to apply for a dental report?

By Email

Please email your request with the required documents to dro@ndcs.com.sg.
Download and complete the (Consent for Release of Dental Information).
Required supporting documents: Please refer to instructions number 1 to 4 in the form for the relevant documents required to be submitted for the application.

Once we have received the consent form and required documents, we will register your request and advise you further on the payment instructions.

Note: Additional charges will be borne by the patient if further tests such as radiological and laboratory investigation procedures are required for the completion of the dental reports.

Timeline for Request Completion

Requests will take four to six weeks to process.

We can only process your application upon fulfilling the verification and receipt of all necessary forms (with all fields completed), supporting documents and payment.

Delivery modes

You may choose to receive your dental report by:

Email: documents encrypted in PDF and sent to the recipient's email address
Self-collection: NRIC to be presented during collection
Collection by representative: An Authorisation Letter with the representative's name and NRIC No. and a copy of the patient's NRIC has to be furnished upon collection
